Insurance Information – We are contracted with most major insurance carriers including Medicare & Medicaid.
Referrals – We require a referral from a primary care provider only when it is required by the insurance carrier.
Copays & Past Due Amounts – We ask for copays at the time of service. If there is any past due amount, it will be requested at the time of service. We kindly require that full payment of past due amounts be received over a period of no more than 90 days. Additionally, we understand that managing healthcare expenses can sometimes be challenging. Thus, we also have healthcare financing options available to assist you.
Self Pay – We do see self-pay patients.
Billing – We will submit it electronically to your insurance company. In order to do that, we will need the insurance card with the subscriber name, date of birth, policy ID and group number. The claim address must be clearly displayed. You are responsible for any co-insurance or deductible amounts per the Explanation of Benefits we receive from your insurance company once the claim has been adjudicated.
Our new patient packets will be automatically e-mailed to you a few days prior to your appointment date. You may choose to fill it out online before your appointment or you may fill it out when you come in for your appointment.
Should you opt to fill out the packet in person, we ask that you arrive twenty minutes before your appointment time. Please bring a photo ID and your insurance card with you to your appointment.
